45 years old female with VIRAL PYREXIA

Chief complaints:
A 45 years old female farmer by occupation came to causality with chief complaints of HEADACHE,
BODY PAINS and FEVER since yesterday night.
And 2 episodes of VOMITING

HISTORY OF PRESENTING ILLNESS:
Pt was apparently asymptomatic 1 day back she developed fever which was associated with body pains and headache 
No history of burning micturation 
No history of chills and rigours
Two episodes of vominting one in night at 10PM and one episode in the next morning 

PAST HISTORY:Pt was taken to private hospital at Nakrekal in the morning No H/O DM, HTN, ASTHMA, CAD, EPILEPS, TB
PERSONAL HISTORY:Diet:Mixed Appetite: NormalSleep: Adequate Bowel and bladder: Regular Addictions: no addictions 
FAMILY HISTORY:No significant family history 
GENERAL EXAMINATION:Pt was C/C/C well oriented with time place and person and moderately built.Pallor:noIcterus: NoCyanosis: NoClubbing :Absent Lymphadenopathy: Absent
VITALS:Heart rate:82bpmBP: 150/100 mmHg RR: 20 cycles per minTemp: 99.6 F
SYSTEMIC EXAMINATION:Pt is conscious CNS:Speech normalNo signs of meningeal irritatingMotor and sensory system- NormalReflexes - presentCranial nerves - intact: CVS: S1 and S2 heardRESP. system:NVBS+, trachea-central ABDOMEN: inspection   shape :scaphoid with no distention   equal symmetrical movements in all quadrants with respiration   No visible pulsations and peristalsis.PALPATION: no hepatomegaly and no splenomegalyASCULTATION: Bowel sounds heard
