       CHIEF COMPLAINTS: A 36  years old female worker by occupation came to causality with cheif complaints of FEVER since 2days along with CHILLS, HEADACHE ,NECKPAIN and 3 episodes of vomiting  HISTORY OF PRESENTING ILLNESS: Pt was apparently asymptomatic 2 day back she developed fever which was associated with chills , headache ,and neck pain  No history of burning micturation  Three episodes of vomiting  PAST HISTORY:  No H/o DM , HTN ,ASTHMA, CAD, EPILEPSY,TB PERSONAL HISTORY: Diet:Mixed Appetite:Normal Sleep:Adequate  Bowel and bladder :Regular  Addictions : No addictions  FAMILY HISTORY: No significant family history  GENERAL EXAMINATION: Pt was C/C/C well oriented with time place and person and moderately built. Pallor:No Icterus: No Cyanosis: No Clubbing :Absent  Lymphadenopathy: Absent VITALS: Heart rate :82bpm Bp:130/80mmHg RR:20 cycles per min Temp: 101.3F SYSTEMIC EXAMINATION: Pt is conscious  CNS:Speech normalNo signs of meningeal irritating Motor and sensory system- Norm

Chief complaints: A 45 years old female farmer by occupation came to causality with chief complaints of HEADACHE, BODY PAINS and FEVER since yesterday night. And 2 episodes of VOMITING HISTORY OF PRESENTING ILLNESS: Pt was apparently asymptomatic 1 day back she developed fever which was associated with body pains and headache  No history of burning micturation  No history of chills and rigours Two episodes of vominting one in night at 10PM and one episode in the next morning  PAST HISTORY: Pt was taken to private hospital at Nakrekal in the morning  No H/O DM, HTN, ASTHMA, CAD, EPILEPS, TB PERSONAL HISTORY: Diet:Mixed  Appetite: Normal Sleep: Adequate  Bowel and bladder: Regular  Addictions: no addictions  FAMILY HISTORY: No significant family history  GENERAL EXAMINATION: Pt was C/C/C well oriented with time place and person and moderately built.
